Wigwe Crash: Pilot Error, Safety Failures Exposed

Fresh revelations from the United States National Transportation Safety Board have pointed to a series of grave safety lapses as the root causes of the helicopter crash that claimed the life of Herbert Wigwe, his wife, son, and three others in February 2024.

In its final report, the agency revealed that the pilot flew into deteriorating weather conditions under visual flight rules, a risky decision that led to a fatal loss of control mid-air.

Beyond the pilot’s misjudgment, investigators exposed deeper cracks within the operator’s safety culture.

The company failed to conduct a proper flight risk analysis, ignored known maintenance issues, and lacked effective safety oversight.

The report concluded that the deadly crash was the result of a dangerous mix of human error and organizational negligence, bringing renewed scrutiny to flight safety practices across the industry.
